Photo Release & Consent

I understand that photos or videos may be taken of me while participating in volunteer activities, and I give my consent for Animal Care League to use these images for marketing, promotional, and other organizational purposes, including but not limited to social media, print materials, and the website.

Non-Disclosure Agreement

As a volunteer with Animal Care League (ACL), I understand that I may have access to confidential information. This may include, but is not limited to:

• Information about ACL clients, donors, staff, and volunteers

• Animal medical and behavioral records

• ACL business, financial, or program information not publicly available

  
  

I agree that:

1. I will not disclose confidential information to anyone outside of ACL.

2. I will not share confidential information with other ACL volunteers or staff who do not need to know it to perform their role.

3. I will not copy, distribute, or electronically transmit ACL’s confidential information without authorization.

4. I understand that improper disclosure of confidential information may result in disciplinary action, including dismissal from volunteering.

Health Insurance

I understand and agree to the terms outlined below in consideration of my voluntary services at Animal Care League.

1. Liability Release: I acknowledge that Animal Care League does not carry liability coverage for volunteers. I hereby release and discharge Animal Care League and its representatives from any and all claims, liabilities, or demands arising out of my voluntary participation.

2. Health Insurance Recommendation: I acknowledge that Animal Care League strongly recommends that volunteers maintain their own health insurance coverage. I understand that Animal Care League does not provide health insurance for volunteers.

Volunteer Agreement: I have read and understand the terms outlined above. By signing the box below, I confirm my agreement to these terms and voluntarily assume any potential risks associated with my volunteer activities.

ACL Liability Waiver

The undersigned assumes any risk of injury which may be incurred as a result of viewing or contact with any or all animals at the Animal Care League shelter and adoption center and/or any and all animals in the custody of any and all Animal Care League representatives.

In the case of persons under eighteen years of age, the undersigned parent or guardian or other adult indicated below assumes any risk of injury which may be incurred to the person under the age of eighteen as a result of viewing or contact with any or all animals at the Animal Care League shelter and adoption center and/or any and all animals in the custody of any and all Animal Care League representatives.

The Animal Care League attempts to screen and restrain animals appropriately, but visitors should be aware that some animals, when introduced to new situations or persons, can behave unpredictably and may, on occasion, become aggressive out of fear or stress.

In consideration of my participation in the activities of Animal Care League, the undersigned, on behalf of him/herself and on behalf of the parents or guardians of any persons who accompany him/her, hereby releases and forever discharges the Animal Care League, its members, officers, directors, employees, agents and representatives from any and all liabilities, actions, causes of action, claims, damages, costs or losses incurred which may arise out of or relate to said participation in any of the activities of the Animal Care League or that may occur while on Animal Care League premises.
